It clearly shows that your IgM is CDC positive for Lyme.
This would more or less be considered an active infection by most mainstream doctors.
IgM is the antibody that initially responds to an infection and then later IgG takes over.
Your IgG is CDC negative and I would assume negative even through Igenex.
I'm not sure about the ELISA (IgM/IgG). Is that the index for a negative? Generally, anything greater than a 1.0 is a positive. Your results may actually be equivocal.
At any rate, it appears that you're positive for Lyme disease. I'm really sorry about that.
When band 39 pops up, that should be a red flag. From my research, it's the one band that shows a current infection.
Apparently, band 39 only show up when the spirochetes are alive (versus dead).
Also, it's specific ONLY to Borrelia burgdorferi.

"With most infections, your immune system first forms IgM antibodies, then in about 2 to 4 weeks, you see IgG antibodies. In some infections, IgG antibodies may be detectable for years.
Because Borrelia burgdorferi is a chronic persistent infection that may last for decades, you would think patients with chronic symptoms would have positive IgG Western blots.
But actually, more IgM blots are positive in chronic borreliosis than IgG. Every time Borrelia burgdorferi reproduces itself, it may stimulate the immune system to form new IgM antibodies.
Some patients have both IgG and IgM blots positive. But if either the IgG or IgM blot is positive, overall it is a positive result.
Response to antibiotics is the same if either is positive, or both. Some antibodies against the borrelia are given more significance if they are IgG versus IgM, or vice versa."

My daughter was also IgM positive at Lab Corp...
our LLMD, a very quiet man, simply held up the test and said, "you are very lucky to have gotten this from this particular lab...
