Can't answer your question because I haven't had a lyme herx yet but I know there are lots of people who have intimate experience with them, but just wanted to say hi!

Hi Mtndog - I, too, was infected in 2003 and that same year I received 4 weeks of IV rocephin.
After about a week on the rocephin I had my first herx and it was HORRENDOUS! I had a 105 fever and was incredibly ill for 4 days.
I have never had a herx as bad as that one since. My opinion is that before you ever hit the spirochetes with antibiotics they're all in the active stage, close to the surface of your body because the body environment is not toxic for them.
When the antibiotics get them the first time it's like a bomb hit them. There are many casualties because they were all out and about doing their business.
Later, when you keep hitting them, they've gone into stealth mode - they change to cyst form, go intracellular, and go deeper into the body. Basically, they're in their bunkers and it's war as they try to evade the toxic antibiotic environment.
The herxes aren't as bad then because you're not killing as many as you did with the first bomb.
A whole arsenal is needed to get them in their hiding places and it takes time. A Lyme doctor is your best bet to help you know how to get to those that are in hiding.
Best wishes to you and I hope you have much success feeling better!

Mtgdog. My first herxes sound a lot like yours with the extreme fatigue and anxiety. The only thing I didn't have was intolerable pain. My joint pain came over a year into my treatment and is not too bad.
Typically your first herx/herxes are the worst. As time goes on they get easier and easier. Now 16 months into treatment I barely notice them.

Yes, that sounds about right to me for your first herx, if you were infected and never treated before!
Best wishes.
