He can expect to have the worst herx that he can imagine. That is if he is also taking abx, if he is not he will herx anyway although milder.
If his ears are poping have him ask the technician to slow down the descent... Sometimes they are not sure just how bad your ear pain is until you tell them.
It is always best to tell them before he is in really bad pain.
If you are near a dive center or a walmart you might be able to pick up some ear planes or ear plugs.( Not the spongy ones the rubber ones ) They will help greatly, he can use them during the descent and they are perfectly safe in the chamber.

Hi Boomerang~ 12 days after starting HBOT, I awoke with flu like symptoms. Backache, extreme fatigue, headache, sore muscles....felt awful. It lasted one day.
I did 40 dives and it did help my stamina a great deal. I'm continuing to do dives once evry 3-4 weeks.
Today I got a lot done....I love days like today....normal seems not so far away....
It took awhile for me to get used to the dives. It was very odd for the first week or so.
Now it's easy.

Thanks so much, guys. I'll talk to them about slowing down the dives. He just said the ear pain was almost unbearable. It really scared me when I heard that.
I had no clue about the ear plugs. Thanks again.

Regarding the ear pain, its not different than an ascent or descent in an airplaine. (although the magnitude of the ascent or descent is much greater). What the previous poster said was correct, have them slow down the descent and ascent. He can also hold his nose and blow gently to help, or to make chewing or yawning motions with his mouth to help facilitate the equalization, just like you would do on an airplane.

The ear pain: GET EAR PLANES at the drugstore. They make a big difference. They are not earplugs. They are specifically for this problem. Take Afrin ahead of time to clear congestion. Swallow water frequently during pressurizing and make sure ears pop. If they don't pop ask the tender/tech to stop increasing the pressure until he can pop his ears and feels better. Learning the vassalva maneuver helps. Ears will usually adjust with all the above. He has to learn how to pop his ears as pressure increases.
