Bring in as detailed a medical history as you can. I recommend a chronological medical history. You'll be surprised at the connections you make by doing this, and realizing when certain symptoms started.
Make sure you bring a pen and paper, and maybe even an extra set of ears if you can. You will get a lot of information, and you want to make sure you have what you need to record the appointment.
If the doctor doesn't raise the following issues, I would ask about them:
1. coinfections, whether the doctor tests for them and if the doctor suspects any based on your symptoms
2. supplements, what supplements the doctor recommends based on your symptoms
3. yeast prevention, especially if you are put on antibiotics, ask about diet, antifungal medications and supplements
4. I'm not sure what your symptoms are, but if you have bad pain, I would ask how the doctor treats pain. If this doctor does not aggressively treat pain, then I would consider finding a pain specialist. Agressive pain treatment does not mean narcotics, but it usually does include a narcotic for break through along with other medications and modalities.

I have not been to an LLMD yet, because I still have not saved the money for it, but the one that I want to go to has an online application that I downloaded and printed out.
I have it all filled out, except for the list of symptoms that I am feeling that day.
I am also keeping a journal of every symptom and how often I get them.
As already mentioned, I have a list of questions ready for him and taking a note pad to list questions as you talk to him is a great idea. One I will have to keep in mind too.
Taking someone else to help remember and understand is also a good idea, as any of us with Lyme know, especially the Neuro-Lyme, we don't always remember so well.

I see it's already been mentioned, but I was going to say, bring a pen and paper to take notes.
Also, having someone there whose mind works isn't a bad idea either. I was glad I had hubby there.
