It can come from many different chronic illnesses. Chronic illness stresses the autonomic nervous system which results in an inability for the body to regulate blood pressure properly.

Aniek - very good point on the chronic illnesses. And since lyme really likes the nervous system, and the adrenals have tons of nerves, it can be a major target of these infectious agents.
There is a pretty good book called "Adrenal Fatigue" - I think the author is named Wilson.
