I am sorry I can't post the whole article but it is 10 pages long!
This looks like a really good article about co-infections. The author appears to be a researcher, not a clinician. But he does talk about treatment, and on page 9 he has an awesome table with choices of treatments that go together with the common coinfections (lyme, bart, babs, myco, earl)
