Sorry to hear your rash is painful. Have you had it checked by a dematologist yet?
I don't think the rash in that picture is painful. BUT.. if someone had shingles THAT would be painful... and deep from what I understand.
OR...
If there was some neuropathy underneath that could explain the pain. I wish I had a better answer.. but right now my suggestion would be to get it checked out by a doctor. It could be viral, or anything really.
I just don't want you guessing.. or us guessing.. especially if you are in pain and waiting for answers.

Ditto on Tincup's suggestion to make sure it is not viral in origin. Recently my husband had a new rash pop up. We thought it was just another Lyme and co. rash, but it turned out to be shingles. It is treated with an anti-viral drug, such as Valtrex. It can also signal that viruses are a factor that must be addressed.
Not saying that this is the cause of your rash--just something to rule out.
