I have generalized brain atrophy...my neuro said my brain looks like that of a 50-60 years old and I am 28.

My congnitive issues are not significant to me compared to neuro pain in my limbs.

Should I take it that this is irreversible? (I actually haven't had an

MRI

in a while, and this symptom was never found in me--just

want to know in case it should ever come up.)

No, it would not be forever. When infections are treated, better blood flow, etc., healing can happen.
