I always seem to realize in retrospect what was happening was a die-off. What exactly happens to most people here with a herx....is it intensification of existing symptoms? appearance of new ones?

I'm on day 5 of Zith/Mep treatment and I'm still waiting for a herx.

I've been on treatment for 20 months of orals and then about a month and 1/2 of IV's and have been through so many herxes along this process and yet I STILL don't think I could accurately answer your questions.
Even within my same body, each time, it pops up differently. Sometimes with something that has neagged me for awhile and will come on much stronger than in the past or else something so totally crazy and I wonder where it came from since it wasn't even a symptom.
It really is a crap shoot and we wait on a time bomb to see what will come next. Sounds like such a fun game. Want to join in on the mystery fun? Ugh!!!!!
But sadly, this question is going to be different with every person here and we do the best we can to support each other over the worse ones and then be support for others when they are the ones having the rougher time.
I've made some wonderful friends and found such caring people here on this board. Sad that you have to all get so sick to have our paths cross in life, but if we have to be sick, I'm glad I'm sick with such wonderful people.
Sorry my answer was absolutely no help what so ever, but good luck to you in your treatment. Keep us posted on how you are doing.

Sometimes I look at "normal" people on the street etc, and think to myself, you would be absolutely horrified if you knew what I've been through/go through.
But then again, one thing life has taught me is that no one is really "normal".

For me a herx is different with every antibiotic
On Bicillin within 8 hours I felt I had been hit by a bus and ached with flu like symptoms
Orals were herxing in 4 hours with different results
IV is very different again and i suppose with Rocepin where it really reaches the brain its a monthly wammy where you cant lift your head off the pillow, intense head pain and pressure and extreme eye pain
Thats why its is necessary to use all of the different ab's in cycles to cover all differnt forms - I still to this day think the MAB/Brorson protocol is one of the best So MAB suggested that we try to be cunning with the Bbs with a pulsing regimen.
He suggested: 2 weeks of Bicillin/Amoxy/Rocephin/ceftin etc - to kill motile spiros then stop
Then Start 2 weeks Macrolides or cyclines for intracellulars-cwd form then stop
Then start 2 weeks Tini/flagyl & Grapfruit Seed to open cysts then stop
2 weeks nothing - to encourage spiros to get active again
then repeat over and over again
and of course making sure those co-infections are treated 1st
