Going on my fifthe relapse of Lyme in 21 years, I decided to make an appointment for the two day evaluation at the Columbia Clinic. I know I have Lyme, all positive tests, symptoms, etc. but I would like to know what to expect from the evaluation and if people who have gone through it found it helpful and it what way ????

Yes, I had an evaluation in 2005. What they do there may have changed. I had lots of cognitive testing, lots of labs run, had a brain spect scan and a couple of hours with Dr. F. (I don't think he sees patients now though).
It was helpful for me.
In your case, you might consider getting tested for other pathogens too, like EBV, HHV-6, Chlamydia pneumonia and enterovirus. You may have more than tick borne pathogens making you ill (I do).
