There is no single test that can tell you that you have Lyme of any kind - acute or chronic. That diagnosis has to be made by a MD, based on clinical history, symptoms and bloodwork typically.
My suggestion is to start a new post in the Seeking a Doctor section with your state and general area in the subject line. You will most likely receive private messages with names of LLMDs - Lyme literate medical doctors - that can help you diagnose and treat chronic Lyme.

Ditto what tcw said.
Your story is not at all unusual. The IDSA treatment guidelines generally don't give high enough doses or for long enough to get rid of lyme.
In addition, the majority of doctors do not test for co-infections and many people end up with lyme and one or more other infections that are passed by the tick.
Treatment for some of these co-infections require different types of medication in order to clear the infection.
Please get to an ILADS lyme literate doctor ASAP. ILADS is an association for doctors who treat chronic lyme disesase. You can look around their site here. www.ilads.org
