I've used both and both contribute in their own way. The liver cleanses get the liver functioning better by cleaning out the sludge. The coffee enemas give you a more immediate, but short-lived boost by making your liver produce glutathione and dump toxins/bile.
I think cleaning the liver is a good thing to do so you can better get rid of the toxins from treatment ... whether treating Lyme, metals, or parasites. It's been a huge boost for me. I've put details of mine in my blog ... all entries on it are titled "liver cleanse".

Milk thistle is good for the liver.
I also have a liver supplement that my doctor really likes, that has artichoke and sarsaparilla in it, nothing else. So maybe look for those ingredients. This one is not really sold in stores.
