how would you determine if it is protozoa or bacteria that is causing chronic illness? through symptoms? what tests should be done?

First, have you tried a gluten-free diet, even for a few days? That can help steer your efforts.
Did you ever find an herbalist on your island or the nearby countries? If you can't find a MD (as you have indicated that your doctor is not much help), please see a certified ND (naturopathic doctor), an L.Ac. (Acupuncturist) or an Herbalist who has a degree from an accredited college. There are many good ones in your part of the world.
You need a doctor to diagnose and determine the best course. Otherwise, you are just shooting in the dark.
There are some things you can do on your own but it is best to have the eyes, ears and experience of a good doctor. -

Many of the symptoms seem to overlap, so it's hard, if not impossible, to say for sure what it is, just based on symptoms.
You need to be tested for all of the co-infections, but even if they're negative, you still may be infected. I originally tested negative for Babesia and for 3 years was not treated for it. Now, 5 years after the tick bite, I finally tested positive.
I started treatment for it last year, initially just w/herbs, like artemesinin, plaquenil boneset tea, etc. The we added roxithromycin, Mepron, & primaquine. (took out the Plaquenil) and about 1 month ago changed to Malarone and choloroquine. For me, this has been the best combination so far.

oh i see.. give me some tips of what should be taken? i'll try to buy later since it has no side-effects right?
what are gluten-free dieT? like milk, is that it?
i'll search for that but i believe there is also rare to have NDs here that graduated from college.
i'll ask my friends if there's any..
sorry for all the posts. i really want to do something for me to feel better. i can no longer work with this. it is ruining my life besides the pain and discomforts i am experiencing.

I just wanted to mention another natural product I've started using recently that has potent antimicrobial effect is cold processed virgin coconut oil. I'm using the one made by Nutiva. It's excellent and really caused a big herx, even with all the antibiotics I'm on. It has to be cold processed though.

Gerard I hope you are taking cover with the next
system moving your way. psano2 is right on with the
coconut oil. Be careful with it if you try it. 1/8
tsp. to start and slowly ramp up. If you get a
bodily reaction it is called herx. Which makes you
feel worse.
