Is there any way to tell if you relapse when you are in remission? What are the signs and symptoms? Blood tests?

I've "relapsed" several times after having some good stretches of time of feeling well. Some times my symptoms would come back gradually and other times they would hit like a freight train.

Blood tests are pretty much worthless. If you're experiencing symptoms - even just a few nagging ones - then you are likely still sick.

Should I get back on antibiotics or just cut back on yeast, sugar, caffeine? I'm beginning to feel the herxing and think it's because I've over indulged in the sweets over the holidays. Any ideas?

I would suspect yeast if you indulged in sugar, but the bacteria like sugar too. I'd try cutting out sugar and maybe taking some anti-candida foods - garlic, onion, coconut oil, etc. and see if it makes you feel worse. If it does, then it's probably candida die off.

If you still have an LLMD, it might be worth talking to him to get a doctor's opinion.
