I have had Lyme symptoms for the past 6 months. I had a Western Blot done recently, which came back IgM positive for 23 and 41 bands, and IgG negative (only 41 band).

I just recently started taking Doxycycline 400mg/day

I am on my 10th day of Doxycycline, and so far I have not felt a change. How long does it usually take before one feels a change?

Really it depends. What were your symptoms and how long have you actually had LD?

For me, I've had Lyme at least 22 years, developed neuro Lyme, so I felt the Doxy immediately (within an hour)

Some people don't herx at all.

Around 6 months ago I got a flu at the end of the summer.

Then a month after that, I developed headaches, neck pain and stiffness, along with back/shoulder pain. I also felt nausea, lost weight and appetite.

I also developed these weird vibrations in my ear after I hear a sound. Also, I feel a pulse in my ear.

I still get continuous headaches (back of head mostly) as well as tremendous neck pain and stiffness.

I got sick about 9.5 months ago, and had symptoms similar to yours, or maybe milder.

I started Ceftin, 500mg twice a day, after 10 weeks, took it for 3 months and never had what seemed like an obvious herx. I did start feeling better by the end of the 3 months, but then relapsed the next day.

I took some of the Buhner herbs for the next month, and never herxed but felt somewhat better by the end of the month.

Then I started doxy just over 3 months ago. I herxed on days 3-13, with the worst of it on days 4-7.

I did not herx again when I doubled my dose of doxy a month later, from 200mg/day to 400mg/day.

I just started Biaxin 4 days ago. I may still herx, but so far I haven't. However, after the second dose, I started to feel a little worse 2 hours after taking it, and a little better again 5 hours after that. That has happened with every dose since then, so I know it is having some effect.

I felt kind of bad the first 2 months on doxy, but started feeling somewhat better, with ups and downs, the third month.

So, based on my personal experience, you might herx, you might not, or you might have a mild one. It might or might not mean anything about whether or not the med is working.

I think if your symptoms are getting better, it's a good sign, since that's the ultimate goal. Still could be you drive the bugs into cysts and feel better from that, but you can deal with it later if it comes back, or your doctor might put you on another drug later on to address that possibility.
