As you have gastritis, it is possible that one drop of GSE/OLE could irritate some tissue/nerves in already irritated tissue. But, with a full glass of water, it's hard to say.
Of the two ingredients, the GSE is far more likely to irritate as a tincture. The alcohol, too, is a consideration. Still, just one drop in a glass of water may produce some sort of herx results but I would not think it would cause liver distress.
I'm thinking it's irritation of the G.I. tract by GSE - and that can cause referred pain - or some degree of herx.
GSE herx is far stronger than OLE herx, in my experience. I cannot do any GSE drops, not even one drop but more due to G.I. tract burning. GSE can be caustic to my tender tissues.
Or it could just be coincidence.
Acetaminophen can cause liver distress, though, in case you are taking that, too. Whatever is going on, I hope you are feeling better. -

Perhaps you are killing a lot of pathogens and that is putting stress on your liver?
Olive leaf supposedly can kill parasites. When I went on a prescription anti-parasitic my liver swelled up and I could hardly walk due to the pain.

- If one drop is too much, you might add more water in a glass jar and divide the doses, keeping the jar in the fridge. Be sure to follow with more water - and maybe take in the middle of a small meal. This will minimize possible irritation of the G.I. tract. -
