Hubby just finished up his Coartem and got started on IV Zithromax this week. Woke up last night with a fever of 100.3 -- first time in a long time. Not sure what to make of that yet. He has been on oral zith off and on for the last couple of years but never IV before.
He is on IV Rocephin, IV Zithromax, factive and pulsing tindamax. Not really sure where we are headed with anything at this point.
But I am pretty sure we have some left over quinine in our stash. May have to give that a try. Will let you know if we do try that combo.
His LLMD appointment has been moved to 10/21 so most likely we will try something on our own between now and then.
His cough is getting worse -- pulmonologist no real help there. Years ago we thought the cough was babesia related. Had been gone for a couple of years and came back when he crashed and was in the hospital early July.

One precaution -- one of the tables listed a pretty high rate of long QT heart rate. Hubby developed palpitations after his first dose of oral zith a couple of years ago -- strangely that symptom went away with either rifampin or factive -- I forget now which but will ask Steve.

17hens, The first regimen more than 5 years ago was Mepron/Zith for months. That did get rid of the severe teeth chattering chills but that was all I noticed.
That was followed by more months of Malarone which didn't seem to do anything.
I had used artemisinin with these also.
The next med I used was Lariam followed by Primaquine which did help and helped me realize Babesia was an issue but relapsed.
Then went on to Alinia for many months. This seemed to keep me from getting worse, but no improvements.
Then I did Riamet/Coartem. That REALLY made me realize Babesia was a major issue. I had a couple of pretty good weeks following that. The best I've been in 6 years. But it didn't last.
Then I did rounds of Chloroquine/Primaquine. The Chloroquine didn't work for me as well as the Riamet so my next plan consisted of riamet followed with Primaquine.
Definite slooooow improvement but relapse follows quickly. I will continue with this for the next 5-6 weeks until I get the above combination.
I initially planned on doing Clindamycin/Quinine combo until I saw this.

I did IV Zith with no noticeable difference at the time but wasn't targeting babs at the time.
The Primaquine seems to be holding the babs in check as it does for relapsing Malaria for the most part but like all of us, that's just not good enough. I want it GONE!
Another interesting thing that put me on the track of Babs being my maybe biggest issue was that for the first time in 5 years, my immune system bloodwork improved after the Coartem.
When I relapsed so did my immune system. Total IgG and the subclasses.
Curious that you mention the cough. When I make some headway with babs the cough nearly disappears. Have had one for years. Much much better now.
I read way back about a vet being diagnosed with pulmonary fibrosis with a poor prognosis and it resolved with babesia treatment. hmmm
Good Luck Bea Know what you mean. If I had a stash right now of Zith and quinine, I'd be starting it yesterday.

Is it possible you might have Bart w/ the Babs? Maybe you'd need to treat both at the same time to get rid of the Babs? Just a thought.

Bea, I noticed the same palps with just initial dosing of Zith, but like your hubby w/factive, Biaxin does not cause any palp issues for me.

For those of you,how do you know that you have bab? because the test is positive?

I was neg thru Igenex test early on.
Then the protocol for it gave me much improvement.
Even with a neg. test if the meds work you could have it.
