The best I can offer is a list of physicians as close to KY as possible. KY isn't exactly a hotbed of Lyme Literate physicians.

I'm in Louisville, KY and if there is a LLMD, I'n not aware of it. I've been praying for one since '99. He'll/she will be here soon!
One must travel to surrounding states about 5-8 hrs one way for LLMD.
Ky is a hotbed of bacteria carrying ticks that make hundreds of thousands of KY people ill...but they are mostly labeled here with MS, ALS, chronic fatigue and fibromyalgia. Oh and the best one mentally not right.
I hope your friend has computer access to be able to gain knowledge and strength from others on this site.
