Has anyone been to Dr. D in North NJ? He is an infectious disease doctor and takes insurance, but I believe is an LLMD too.
I've been sick for over 2 years, have been in treatment, but recently plateaued. I still can't walk much, can't drive because my vision is affected, air hunger, fatigue...etc, etc

Generally ID docs are the enemies of lyme patients. They are members of the Infectious Diseases Society of America (IDSA) which says that chronic lyme does not exist. They will not treat lyme long-term.
Also, any doc who takes insurance cannot spend the time necessary to properly evaluate and treat a lyme patient. Insurance reimbursement schedules mean that the first appointment cannot be more than 30 minutes and followup no more than 10 minutes.
A lyme specialist spends at least 1 hour with the patient at the first appointment. The patient input is essential to crafting the treatment plan. Follow up appointments are 30 minutes because patient report of symptoms and changes in symptoms once therapy is initiated are essential to know how treatment has to be changed to achieve success.
So, just don't go in there with your hopes up. It doesn't sound like this is going to be fruitful.
With your symptoms, you really should try to get in with the very best lyme doctor you can possibly afford.
I can give you a name outside the D.C. area if you are interested. You could take the train to D.C. and then the subway to within a couple miles of his office. Then, a short cab ride. And, you can do telephone or Skype appointments for 2 months so you only have to appear in person once every 3 months.
