First, a huge thanks to everyone for all the help you've given already. My friend is now on Dr. J's waiting list, but since it may take a while to get an appointment, I'm still researching other possibilities. Dr. C in Tampa's name has come up. My parents and sister saw him for several months in winter 2004. My parents then returned to Cape Cod for the summer, during which time they got a letter from Dr. C that he was dropping them from his practice.
The reason given was his own health, but I am not sure that is true as he has a reputation for abandoning patients.
Also, if you do not live in his immediate area and he recommends IV, you will need to find a doctor local to you who will rx and administer it. My father had to jump through hoops to find someone local to them to rx Rocephin, and even once that was done, that doctor would not rx it the way that Dr. C recommended, would only do it for 28 days, and pronounced "cured".
Dr. C will, though, provide rx'es for orals even if you live outside of his immediate area.
If you are not able to find someone else, and he will take you as a patient, you should see him, but, based on what he did to my parents and sister, and what I heard from another ex-patient, you should be prepared for him to drop you.
Perhaps if you see him regularly, it won't be a problem. My parents are only there six months of the year, so perhaps that is the real reason he abandoned them.

I think this is his way of avoiding problems for "overtreating" Lyme. Has been doing this for several years, and I don't think it had anything to do with your parents only being in state for part of the year.
Viva, think there is a LLMD in Mobile, have you contacted the AL support people? If interested in this location, might try them.
Dr. J also dumps some people after a certain amount of treatment, not sure of his rules, no one tells you this upfront.
