Hello, and is anyone aware of a spreadsheet template showing clusters of symptoms to be rated on objective number scales. This is to meet my doctor's request for more tangible and objective feedback of progress than simple journalling. Thanks!

Hi, Janette!

My doc suggested that I make a chart to make recording my symptoms easier. I am absolutely awful at journaling.

She told me to take a grid and list the symptoms that I usually get down the left side as labels.

Then mark the top of each column with the date.

Each day, give each symptom a rating according to its intensity that day ranging from 0 to 5. 0=not at all and 5=very intense.

This is a bit more objective and is a faster read than a journal.
