Our daughter has a PPO health plan through United Health Care of California. They refused to pay for IV antibiotics beyond 30 days.

I've had no problems with a Cigna PPO. But: I'm not on IV, and it's a self-funded plan through my employer so the employer has more control. The drug plan is seperate from the PPO.
If you are purchasing on your own, they will design the plan to save themselves money. Remember that.
