The Impact of Chronic Lyme Disease on Work
Employees with Chronic Lyme Disease understand what a challenge it is to work a "normal" job. Because the disease causes variable physical, mental, and emotional symptoms, it can be difficult to work regular work hours or perform consistently. Unfortunately, most employers do not understand that Chronic Lyme Disease can cause cycling symptoms that affect the mind, body, and emotions.

I've created an anonymous survey in Websurveyor that contains demographic, open-ended, and opinion scale (known as Likert scale) questions. The goal of the survey is to understand exactly how Lyme Disease affects one's ability to work. The results of my research will be made available on this Blog and submitted for publication. If enough people take the survey, maybe we can influence policy on workplace accommodations for those with Chronic Lyme Disease!

To date, there has been no study conducted on this topic. Please help by completing the survey today! You can access the survey by clicking on Take the Survey. THANKS!

This Blog is linked to a research study I'm conducting at James Madison University on the impact of Chronic Lyme Disease on learning and performance at work. I am interested in this topic because I have witnessed the many ways the disease impacts a person's ability to think, learn, plan, participate in social interactions, and engage in physical tasks. I am undertaking this study because I think it is time for employers and the general public to understand exactly how this disease impacts patients' lives - especially their ability to earn a livelihood.

SURVEY CONSENT FORM

Identification of Investigators & Purpose of Study
You are being asked to participate in an anonymous research study conducted by Diane M. Wilcox, Ph.D. at James Madison University. The purpose of this study is to examine the impact of chronic Lyme Disease on basic workplace competencies such as learning, thinking, writing, and interacting with co-workers.

Potential Risks & Benefits
The investigator does not perceive more than minimal risks from your involvement in this study. No personal information will be asked and responses will be saved to a data set, so it will not be possible to link responses to an individual. Responses will be categorized and totaled to present a picture of how the illness affects a person's ability to perform at work. The main benefit of this study is that participants will have access to study results at http://www.lyme-and-work.blogspot.com This goal of this study is to influence policy on workplace accommodations at the state and federal level.

Research Procedures
This study consists of an online survey administered to individual participants using Websurveyor. The link for the survey is being sent to Lyme Disease support groups and posted on a Blog entitled ``The Impact of Lyme Disease on Workplace Performance.'' You will be asked to answer a series of questions about your experience in working with chronic Lyme Disease.

Confidentiality
While individual responses are anonymously obtained and recorded online through Websurveyor, data are kept in the strictest confidence. No identifiable information will be collected from the participant and no identifiable responses will be presented in the final form of this study. All data will be stored in a secure location only accessible to the researcher. The researcher retains the right to use and publish non-identifiable data. Final aggregate results will be made available to participants upon request. It will also be made available to Lyme Disease support groups and associations, published in trade and professional journals, and presented at conferences.

Participation & Withdrawal
Your participation is entirely voluntary. You are free to choose not to participate. Should you choose to participate, you can withdraw at any time without consequences of any kind. However, once your anonymous responses have been submitted, there is no way to extract your responses from the data set.

Questions
If you have any questions about the study, please contact Diane M. Wilcox, Ph.D. at [email protected] .

GIVING OF CONSENT
I have read this consent and I understand what is being requested of me as a participant in this study. I certify that I am at least 18 years of age. By clicking on the link below, and completing and submitting this ``anonymous'' online survey, I am consenting to participate in this research.

I am too .. just to see what they turn up...
I have been alwys interested in how badly this hits the lives of everyone...
I know it hit my family hard.. and with 2 lives lost in a years time .. both members the male role models it makes life harder to direct my sons who have no male role models to look up to and their father.. was not much in the way of a father to them.. I gather the lyme had a lot to do with that....
but anyway... they missed out and it is sad they now have to listen to cackling old hens all the time... and have no males to model after....
such is life I am sure they arent the only ones who had to model after females.. LOL

but anyway .. this should not be.. ..
and since this disease robbed them of their male role models...

I have to find out as much as I can in hopes to give them more quality in life than us women have.. which isn't much..
thanks all,,
LindaD
