I provide a 1-page info sheet (that I keep on my computer, and adjust as necessary). Doctors and nurses love this. You make a copy for yourself and one for the dr. and you can both look at it together. Just be sure to keep the date current.
Here is my template. (Adjust your page layout so it all can be printed on one page. 10pt font works, and you need to highlight the headings.) Just copy and past this template and fill in your info!

Emergency contacts + phone numbers - include all people you want be contacted
Today's date:
Current medications: (as of current date) Prescription
Non-prescription (over the counter, plus herbs and vitamins)
Immunizations (tetanus, flu, etc.)
Allergy to medications I have shown negative reactions to the following medications: List medication, date, and your specific bad reaction to each.
Recent procedures (mammogram, colonoscopy,flu shots etc.). - name and date of each, Surgeries
Recent Medical Problems: Include problem and date of onset to date cured, or to current date and what remedy was used. (example: back injury, and any other things that you have had to deal with over your medical life
Primary Physician: Name, Phone & Fax numbers Specialists: (cardiologist, chiropractor etc.) names with phone and fax numbers Dentist: name, phone and fax numbers Primary Insurance: name and ID number Secondary Insurance: Name and ID number Power of Attorney: name and phone number Living Will: (how to get that info)

oops, forgot to rinse the shampoo out this morning. OMG I only put mascara on one eye. You want me there in WHAT??? 10 minutes?? It takes 4 trips to the car, 2 searches for the spare house key in the garage, where's my purse? I'll be there in 2 hours!
