Hello, and is anyone aware of a spreadsheet style template of symptom clusters for monitoring symptoms on a number scale, to provide doctors with objective and tangible treatment feedback?

If you have Microsoft Works, they have several spreadsheets you can customize.

I am looking for the MS Works or Excel symptom monitoring spreadsheet. My husband had had LD for at least 5 years and is having a bad time of it right now. I want to help him log his symptoms to see if there is a pattern. Any help would be appreciated!
