New Jersey Folks please jump on this VERY IMPORTANT REQUEST by PAT SMITH!!!

WHO: NJ residents, patients who see NJ doctors, or NJ health care providers.

WHAT: One page letters *describing your Lyme disease insurance experiences and/or experiences trying to get diagnosed or treated or find a doctor, other doctor/Lyme issues

*NO DOC NAMES IN LETTERS.

*You MUST include a statement giving LDA permission to share the letter with anyone.

*Must have name, address on letter

WHEN: By December 11, 2004
