I ordered a newsletter devoted to children and Lyme from Lyme Times at calda.org. I think it was 5 or 10 dollars.
I wonder this all the time as my 5 year old nephew is going into treatment. Have you tried searching some old posts?
Dr. C. in PA has written some good articles on Lyme in Children but doesn't go into specific meds or dosage.

I don't believe Dr. B has guildelines for children.
I know in my daughters case (Lyme and Babs) Dr. J treats the Babs with Mepron & Zith, then started after the Lyme with Tindamax 4 months later (our next visit).
Slow and steady, she has been on this for 14 months.
