30 days refrigerated (kept dark) and 6 months frozen.
After that, there have been no tests and it may not be good, and certainly may not be full strength.

I used to work in a pharmacy that made I.V. meds. There are a lot of factors that determine an expiration date.
If it's the type of system where a vial is attached to a bag and you break a stem and mix just before you use it, then it's good for longer than a bag that the pharmacy made.
Also, there are bags that are pre-made by the pharmaceutical company. These are stable for a longer period of time than a bag that the pharmacy mixed.
Also the solution and volume used to mix the medication matters. So, it's not a really easy question to answer.
Oh yea, almost forgot, there are new federal laws prohibiting long expiration dates on things mixed by a pharmacy.

We just went through the same thing with IV Doxy. It was in the frig for a week. Pharmacist at Infuserve said "toss it. Its no good"

IV doxy doesn't last long mixed. Rocephin lasts longer. Although, as susieq posted, I guess it depends on different factors.
I get mine in syringes from Infuserve, and what I mentioned above is what they told me and what they print on the syringes. Infuserve is very careful and knowledable about these issues.
