Saw a couple of people asking where to find supporting documentation to address an insurance denial, so dug out some documents I created a while back and put them on the Maryland Lyme website under "Help For You"- Insurance Issues.
Documents are located/linked at the bottom of the page and will fit various situations, depending on your specific needs.
Please remember to adjust the introductions on each page to fit YOUR circumstances. Use any/all of it, whatever you think will be helpful.

Also forgot to say there are still some sample letters to use and instructions to file complaints to the Attorney General in your State (in simple language for those who find it difficult to concentrate).
